OWLET APPLICATION
We provide Owlet Dream Socks to infants who are at high risk for SIDS. To apply, please first download the High Risk Screener below and have your doctor complete this form. Attach this completed form along with all the questions for the Owlet Application to apply for a Dream Sock.
Due to limited funding, we are only able to accept a limited number of applicants at this time. Each application is reviewed throughly. Applications are only accepted within 30 days prior to your due date OR 3 months after your due date. Please click the application link below to view other requirements to apply.
Applications are reviewed monthly. Once applications have been accepted we will notify recipients via email
If your application is not accepted, you will not be notified. You are welcome to reapply the following month as long as your baby still meets all requirements.

SIDS & SAFETY SEMINAR
We LOVE the Owlet Sock! But, did you know that the Owlet sock isn't enough? You get your first alarm from the Owlet-now what? Just as important as the Owlet is preparing yourself for an emergency!
Join our SIDS & Safety Seminar. Classes are held monthly and the cost is $150. You will receive education on SIDS including what SIDS is, recent advances in SIDS research, SIDS prevention, safe sleep practices, and CPR certification. Upon completion, you will also receive an Owlet sock for your baby. Having twins? Please indicate in your course registration that you are expecting twins.
COURSE VALUE: This offer, including the course, CPR certification and the Owlet device, is valued at over $500. Our nonprofit organization offers all of this at a discounted price of $150.
